*   Application Forms
  • Member Personal Details Change Form

    • Complete this form to change your Title, Date of Birth, Surname, Given Names, Address, Contact Numbers and email address.
    If you are changing your name or date of birth, you must also provide supporting documentation (see form for more details).
  • Benefit Transfer Form

    • Complete this form if you have superannuation in other funds and you want it transferred to your SMERF account. This form authorises SMERF to approach your other superannuation fund(s) to arrange for the transfer of your other account(s) to your SMERF account.
  • Trustee Application Form

    • This form is for use by Trustees who wish to use SMERF as their nominated Eligible Rollover Fund.
    Mail the completed form to SMERF, PO Box N835, Grosvenor Place, NSW, 1220.
  • Nomination of Beneficiary Form

    • Complete this form if you would like to advise SMERF of your preferred beneficiaries. This nomination will replace any existing beneficiaries already nominated.
