- Member Personal Details Change Form
Complete this form to change your Title, Date of Birth, Surname, Given Names, Address, Contact Numbers and email address.
If you are changing your name or date of birth, you must also provide supporting documentation. (see form for more details). - TFN Collection Form
Complete this form to provide your Tax File Number to SMERF.
- Benefit Transfer Form
Complete this form if you have superannuation in other funds and you want it transferred to your SMERF account. This form authorises SMERF to approach your other superannuation fund(s) to arrange for the transfer of your other account(s) to your SMERF account.
- Benefit Payment Request
Complete this form to request partial or full payment of your superannuation benefit.
- Trustee Application Form
This form is for use by Trustees who wish to use SMERF as their nominated Eligible Rollover Fund.
Mail the completed form to SMERF, Locked Bag 8840, WOLLONGONG NSW 2500. -
Nomination of Beneficiary Form
Complete this form if you would like to advise SMERF of your preferred beneficiaries. This nomination will replace any existing beneficiaries already nominated.
For further information regarding preferred beneficiaries, you should refer to our Product Disclosure Statement.
